591-1-1-.12(4) · Rule 591-1-1-.12(4) requires that equipment and furniture is secured if it is of a weight or mass that could cause injury from tipping, falling, or being pulled or pushed over.
During an investigation, it was determined that on September 8, 2025, at approximately 3:26 p.m., a basketball goal on the playground did not contain an adequate amount of sand or water to be securely anchored. A 4-year-old child was observed climbing the basketball goal post and when the teacher approached the child requesting them to get down, the child jumped off the equipment, causing the goal post to fall and strike them in the right eye. Professional medical attention was obtained as a result of the injury.

591-1-1-.12(2) · Rule 591-1-1-.12(2) requires that all equipment and furniture be free from hazardous conditions such as, but not limited to, sharp rough edges or toxic paint; and be kept clean.
During an investigation, it was determined that on September 8, 2025, at approximately 3:26 p.m., a 4-year-old child was observed climbing a basketball goal post located on the playground. The child jumped off the equipment, causing the goal post to fall and strike them in the eye. The basketball goal was not in good repair, as the rim lacked the required netting and exposed wire was present. The exposed wire caused an injury to the right eye. Professional medical attention was obtained as a result of the injury.

Fencing material shall not present a hazard to children and shall be maintained so as to prevent children from leaving the playground area by any means other than through an approved access route. During a Complaint Investigation & Monitoring Visit conducted on July 3, 2024 ; It was determined based on observation that there was approximately a four and a half inch gap at the bottom of the gate and the latch on the fence between playground A and playground B.

During a Complaint Investigation & Monitoring Visit conducted on July 3, 2024 ; It was determined based on observation that the following hazards were observed: B/2R - Chipping paint in the block area. E/Mid/1R - A broken blue locker door. - Staples throughout. - Missing floor tile near the right back wall. - A metal piece protruding from the back wall near the back door. H/1L - Chipped paint on the wall in the block area. - Chipped dry wall on the left side of the back window in block area. - A broken top corner of the wooden block shelf. I/2L - Missing wall baseboard in the girls and boys restroom.

During a Licensing Study conducted on April 9, 2025 ; It was determined based on observation that the following hazards were observed: C/3R - The sink in the classroom was not operable. H/1L - Flooring material was missing underneath the sink. G/Mid 3R - There was a large water stain on the ceiling in the boy's restroom. J/3L - The plastic strip was broken on the top corner of the wooden shelf in front of the classroom.

During a Licensing Study conducted on April 16, 2024 ; It was determined based on observation that the following potential hazards were present: Playground A: -There was a small active ant bed next to the entrance gate. Playground B: -There was a presence of wasps and wasp nest under the wooden gazebo. Playground C: -There were mushrooms near to middle of the back fence. -There were three medium sized active ant beds along the left sidewalk near the porch. Playground D: -The black barrier around the left side of the playground structure was cracked. -There was a presence of bumblebees near the wooden gazebo.

Playground D: - Moldy water in the sensory table.
