    591-1-1-.32(7) · Rule 591-1-1-.32(7) requires that children be supervised at all times appropriate to the individual age, needs and capabilities of each child. Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. "Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers and Students-in-Training. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in- Training, and provide timely attention to the children's actions and needs. Staff shall be attentive and participating with all children during mealtimes and shall be seated within an arm’s length away from children thirty-six (36) months of age and younger.

    During a Monitoring Visit conducted on November 8, 2022; It was determined based on observation that a staff member was observed to have walked to the front entrance leaving classroom A with no staff and eleven children who were three-years-old for approximately one minute. It was further determined based on observation that a staff member was observed to have walked across the hall into another classroom leaving classroom B with no staff and two children who were one-year-old and eight children who were two-years-old for approximately one minute. It was also determined that classroom D housing seven children who were four-years-old and two children who were eight-years-old did not have a staff member and were unsupervised for at least thirty minutes. There was no additional staff to provide coverage for the operating classrooms.

    During a Licensing Study conducted on May 2, 2022; It was determined based on observation that plastic bags were located in the children's containers and baby wipes were located on the diapering station in, Room B, accessible to children. It was further determined based on observation that baby wipes were located on the diapering station and in the children's containers in, Room C, accessible to children.

B: Left Side 1R: -There were plastic bags and hand sanitizer on the diaper changing table. -There was an open storage closet with a broom and laundry detergent. D: Right Side 1R: -There was a pair of adult scissors and two cords on the table. -There was a broom, dust pan, and Swiffer next to the cots.
