During an investigation, it was determined that on August 15, 2025, a child was instructed by a staff member to close a closet door, resulting in a finger injury to another child in the classroom. As the staff member was in the closet retrieving chairs, two 1-year-old children were standing in front of the closet door. One of the children placed their fingers in the door jamb near the hinges. The staff member exited the closet carrying the chairs and told the other child to close the door. The child followed the staff member’s instructions and closed the door, resulting in an injury to the child with their fingers in the door jamb. The child received professional medical attention for a finger injury.

591-1-1-.32(7) · Rule 591-1-1-.32(7) requires that children be supervised at all times appropriate to the individual age, needs and capabilities of each child. Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. During an investigation, it was determined that on August 15, 2025, adequate supervision was not provided when a 1-year-old child sustained a finger injury after a door was closed on the child’s hand. As a staff member was in the closet retrieving chairs, two 1-year-old children were standing in front of the closet door. One of the children placed their fingers in the door jamb near the hinges. The staff member exited the closet carrying the chairs and told the other child to close the door. The child followed the staff member’s instructions and closed the door, resulting in an injury to the child with their fingers in the door jamb. The staff member was not aware of the position of the child’s hand and did not ensure that the door jamb was clear before the door was closed. The child received professional medical attention for a finger injury.

During an investigation, it was determined that on August 1, 2024, a 5-year-old child was left alone and unsupervised in a childcare vehicle for approximately 20 minutes. On the date of the incident, staff transported nine school age children from an elementary school to the center. Upon arrival at the center, the 5-year-old child was asleep on the vehicle and did not exit the vehicle with the other children. Staff were unaware that a child was still in the vehicle until a parent reported to staff that they heard the 5-year-old child in the parking lot. Staff went to the parking lot where they met the child, who had exited the vehicle and was walking towards the center.

During an Incident Investigation Closure conducted on March 27, 2023 ; It was determined based on an investigation that on May 12, 2022 at 9:42 a.m., center staff failed to provide supervision-watchful oversight when a four-year-old child sustained an injury after they fell from playground equipment that was used inappropriately. The child flipped the car on its side, stood on it and jumped off of the car backwards. The child had to receive professional medical attention as a result.

During an investigation conducted on October 14, 2021, it was determined that on September 29, 2021, a three-year- old child sustained an injury to the nose when the child fell hitting their nose on a table after getting up from eating breakfast. Six children were eating breakfast, the staff person positioned themselves across the room and was looking down at something in their hands. There were eight other children engaged in free play. The three-year-old child got up from the breakfast table and stumbled hitting their nose on another table. The child walked to the staff person rubbing their nose. The staff person walked by the child and did not stop to check on them. The staff person failed to participate with the children during mealtime. Additionally, the staff person was not aware of the incident until the parent made center staff aware of the injury to the child’s nose. Professional medical attention was received.
