During an investigation, it was determined that on May 13, 2025, at 2:31 p.m., a 5-year-old child was left unattended on the center vehicle for approximately three hours. According to Weather.com the high temperature for that day was 83 degrees. During the unloading procedure of the children from the vehicle, center staff failed to conduct a face-to- name check as the children exited the vehicle. Center staff escorted the children into the building and conducted a head count as the children entered the building. At approximately 5:30 p.m., a family friend arrived at the center to pick up the 5-year-old child. The individual was informed that the 5-year-old child was not present at the facility. Center staff then conducted a check of the vehicle and found the missing 5-year-old on the floor of the vehicle under the seat. The child was observed to be hot, sweaty, and thirsty when found unsupervised on the vehicle. The staff failed to provide adequate supervision and watchful oversight when the staff members were unaware that the 5-year- old child was missing and left on the center’s vehicle for approximately three hours.

The vehicle arrived at the center at 2:31 p.m. after picking up the school-age children from their elementary school. The required first check of the vehicle was not conducted by the staff member present on the vehicle before they exited the vehicle and entered the center. The center staff failed to complete the first check of the vehicle which resulted in the 5-year- old child being left unattended in the vehicle for three hours.

During an investigation, it was determined that on May 13, 2025, a second check of the vehicle was not completed when a 5-year-old child was left unattended on the center vehicle for approximately three hours. The vehicle arrived at the center at 2:31 p.m. Transportation logs indicated that a second check of the vehicle was conducted for the day, accounting for all children exiting the vehicle. Staff stated they looked over the seats of the vehicle as a second check, but staff did not physically walk through the entire vehicle and thoroughly check each seat, including underneath the seats as required. At approximately 6:00 p.m. the six-year-old was found unattended on the vehicle by a staff member who checked the vehicle after the child was not located in the center for dismissal to a family friend. The transportation checklist was completed and incorrectly marked, indicating that a second check of the vehicle was done. The center staff failed to complete the second check of the vehicle which resulted in the 5-year-old child being left unattended in the vehicle for three hours.

The 5-year-old child did not exit the vehicle when the vehicle arrived at the center. The transportation checklist was completed and incorrectly marked indicating the child unloaded the vehicle when it returned to the center. Approximately three hours later, a staff member found the 5-year-old child in the vehicle unattended when a family friend arrived at the center for afternoon pick-up. The center staff failed to account for each child transported for the unloading of the center’s vehicle.
