During an incident investigation and follow-up visit conducted on December 23, 2024, it was determined based on an investigation that staff failed to provide adequate supervision on December 6, 2024, resulting in a two- year-old sustaining an injury to the right arm. The two-year-old child stood on a large foam block and fell, resulting in the injured arm. During the incident staff were sitting on a bench in the room and not in a position to promptly attend to the child as required. The injury required professional medical attention.

During an investigation, it was determined that on June 13, 2024, at 3:38 p.m., a two-year-old child was pushed off a step stool by another child while trying to wash their hands. The staff member went to the door to speak with the office administrator while the children were at the sink handwashing. According to video footage, the staff member had their back to the children when speaking to the office administrator. At the time of the incident, there was an additional child that climbed up the step stool to wash their hands. The two-year-old child was pulled back by another child causing both children to fall off the step stool. The two-year-old child hit their head on the corner of a bookshelf. The child sustained a laceration to the head that required professional medical attention and received three staples.
