During a LS POI Follow Up conducted on July 20, 2022 ; It was determined based on the Consultant's observation that a wooden fence plank was broken on the far back side center left of the toddler and two-year-old playground and a 5 and one-half inch by 20 inch gap was present.

During a Monitoring Visit conducted on March 9, 2023 ; It was determined based on observation that there was a free standing fence post next to the fence on the right hand side of Main B playground and a five inch gap was present. Further, there was a fence panel that was unattached at the top of the panel on the right side of the Main B playground towards the end nearest to the road and there was an exposed nail. Also, there was an exposed screw on the fourth fence panel from the right that was next to the road.

During a Complaint Closure conducted on June 30, 2022 ; It was determined that on April 20, 2022, at approximately 3:30 p.m., watchful oversight was not provided when a five-year-old child was left on a center vehicle alone and unsupervised for an undetermined amount of time when the driver of the vehicle exited the vehicle and took their food and personal belongings into the center and left the child unattended and unsupervised on the vehicle. A parent picking up their child retrieved the child and returned them to center staff. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During an incident investigation, it was determined that on April 20, 2022, at approximately 3:30 p.m., watchful oversight was not provided when a five-year-old child was left on a center vehicle unattended and unsupervised for an undetermined period of time between one and 27 minutes. Another child alerted a parent that the five-year- old child was on the vehicle. That parent retrieved the five-year-old child from the vehicle and carried the child to the front door of the center and released the child to center staff.

During an investigation, it was determined that on April 20, 2022, at approximately 3:30 p.m., the center failed to report an incident to the Department when a five-year-old child was left on the center’s transportation vehicle for an undetermined period of time between one and 27 minutes.

During an investigation, it was determined that on April 20, 2022, center staff erroneously completed the checklist to indicate all children had exited the vehicle when a five-year-old child remained on the vehicle, asleep. As a result, a five-year-old child was left unsupervised on a center vehicle following an unscheduled and unauthorized trip to McDonald’s when the child had fallen asleep on the vehicle following routine transportation from the child’s elementary school. The Staff marked the child as exiting the vehicle, but this was a false mark as the child did not exit the vehicle until the child was found by a parent after staff exited the vehicle and had gone into the center, leaving the child alone on the vehicle for an undetermined amount of time. Additionally, three children were marked as being absent, however records indicated that the children were present.

As a result, a five-year- old child was left unsupervised in a center vehicle for an undetermined period of time between one and 27 minutes.
