During a Licensing Study conducted on August 20, 2024 ; It was determined based on observation that the exhaust fan in classroom F-2R was not working.

During a POI Follow Up conducted on October 17, 2024 ; It was determined based on observation that the exhaust fan in classroom F-2R was not working. Director could not provide consultant with any proof of a work order being submitted to fix the exhaust fan.

During a Licensing Study conducted on August 20, 2024 ; It was determined based on observation in classroom B-2L, there were rodent droppings in the storage closet near the sleeping mats. It was also observed in classroom B-2L that the ceiling appeared to have black mold. Water marks could be seen throughout the ceiling, and there were spider webs located in the back right corner. It was further observed that the bathrooms in classrooms A-1L, B-2L, E-3R, and F-2R had a large amount of dust covering the exhaust fans.

It was further observed that the bathrooms in classrooms A-1L, and B-2L had dust covering the exhaust fans.

It was also observed that the wires connecting to the gate were detached from the pole and torn. It was also observed that the wires on the fence located at the back right of playground A front had an opening measuring six inches across that lead to the woods.

It was also observed that the wires connecting to the gate were torn and detached from the pole.The gate wires were also detached from the ground and bent. Consultant also observed the back gate located next to the front entrance door was opened while children were playing.
