591-1-1-.26(8) · Rule 591-1-1-.26(8) requires climbing and swinging equipment to have a resilient surface beneath the equipment and the fall zone from such equipment must be adequately maintained by the Center to assure continuing resiliency.

During a Licensing Study conducted on June 27, 2023 ; It was determined based on observation that the resilient surface on all playgrounds measured less than one inch under fall zones including swings and climbing equipment when six inches was required. Exhibit "A"

Hygienic measures such as, but not limited to, screened windows and proper waste disposal procedures shall be utilized to minimize the presence of rodents, flies, roaches and other vermin at the Center.

During a Monitoring Visit conducted on December 15, 2022 ; It was determined based on observation that classrooms A, B, C. D, E, F, and G had ceiling tiles with water stains.

Between each diaper change, the diaper changing surface shall be cleaned with a disinfectant and dried with a single-use disposable towel.

During a Monitoring Visit conducted on December 15, 2022 ; It was determined based on observation that in classroom C and D the changing tables had chipped paint revealing the wood underneath, making the surface not nonporous as required.
