During an investigation, it was determined that on March 6, 2026, at 3:00 p.m., a 4-year-old child was left alone and unattended on the center's vehicle for approximately 2 hours 40 minutes. During the afternoon transportation, there were two school aged and four 4-year-old children, with two staff members that arrived at the center from an afternoon bus route to Mount Yonah Elementary and White County Head Start. Staff Member #1 was positioned in the driver’s seat, while Staff Member #2 was positioned in the front passenger seat. The 4-year-old child was positioned in the front row of the vehicle, directly behind the driver’s seat, and buckled in a car seat. The center’s transportation records indicated that the child was marked as accounted for loading at 2:18 p.m. and unloading at 3 p.m., however, the child did not exit the vehicle, when the five other children exited the vehicle. At approximately 5:40 p.m., the child was discovered on the vehicle after the parent’s arrival at the center during pick up time. When discovered, the child’s face appeared to be flushed and red in color. The staff member failed to provide adequate supervision and watchful oversight when center staff were unaware that the 4-year-old child was missing and left in the center van for 2 hours 40 minutes.

591-1-1-.36(7)(d)2 · Rule 591-1-1-.36(7)(d)2 requires that the second designated Staff person conduct a check of the vehicle immediately upon the completion of the first check of the vehicle. The responsible person shall physically walk through the entire vehicle; visually inspect all seat surfaces, under all seats and in all compartments or recesses in the vehicle's interior; and sign the passenger transportation checklist(s), indicating all of the children have exited the vehicle. There shall be continuous watchful oversight of the vehicle between the first check and second check.
During an investigation, it was determined that on March 6, 2026, a second check of the vehicle was not completed when a 4-year-old child was left unattended on the childcare van for approximately 2 hours 40 minutes. The transportation vehicle arrived at the center at 3 p.m. According to transportation logs, staff Member #2 reported that name-to-face checks were conducted both at the school and again at the center before exiting the vehicle for the day. The staff member indicated that a second check of the vehicle was conducted for the day, accounting for all children exiting the vehicle. The driver parked the vehicle and immediately exited the vehicle without conducting a check by physically walking through the vehicle and inspecting the seats and underneath the seats. The driver indicated that a second check was completed, and at approximately 5:42 p.m. the 4-year-old child was found on the vehicle by a different staff member after the parent’s arrival at the center during pick up time. Staff incorrectly completed the transportation checklist which was marked indicating that a second check of the vehicle was done when a 4-year-old child was left unattended on the vehicle for approximately 2 hours 40 minutes.

591-1-1-.36(7)(c)2 · Rule 591-1-1-.36(7)(c)2 requires that the driver or other designated person shall immediately document in writing, with a check or other mark/symbol to account for each child listed on the passenger transportation checklist each time a child enters and exits the vehicle. During an investigation, it was determined that on March 6, 2026, at 3 p.m., the center staff failed to ensure that a 4-year-old child was comfortable when the child was left alone on the center’s vehicle for approximately 2 hours 40 minutes. The child was described as having “rosy cheeks” and requested water when they were removed from the vehicle. The outdoor temperature was approximately 66-67 degrees during the time the child was on the vehicle.
