During an investigation, it was determined that on November 10, 2025, a 1-year-old child slid down a slide on the playground in the lap of a staff member, and sustained an injury to the leg. Staff did not protect the child from unintentional harm as required. The injury required professional medical attention.

During an investigation, it was determined that on November 10, 2025, a 1-year-old child sustained an injury to the leg while on the playground and the parents were not notified immediately as required. The child was observed not to walk on the injured leg and cried throughout the day. The injury happened between 2:30 p.m.and 3:00 p.m. and the parents were not notified until pick up at 5:21 p.m.

During an investigation, it was determined that on November 10, 2025, a 1-year-old child sustained an injury to the leg that required professional medical attention. During an investigation, it was determined that in May 2022, video footage showed a staff person yelling at a child due to a toileting accident. The second video showed the staff person belittling the child because of the toileting accident.
