During an incidnent investigation conducted on November 1, 2021, it was determined based on investigative findings that on October 21, 2021, a four-year-old child fell from a slide and sustained an injury to their arm that required professional medical attention. The evidence to include staff statements and interviews, video footage, and diagrams drawn by involved staff members, established that staff were not able to provide immediate intervention when the child was observed to have been using the slide in an inappropriate manner. Supervison and watchful oversight were not demonstrated in a manner that could have prevented the incident from having occurred.
