During a Licensing Study conducted on September 27, 2023 ; It was determined based on a review of records that the center did not conduct a fire drill for the month of August 2023. The center did not conduct a check of the smoke detector for the months of January- July 2023. The center did not conduct a check of the fire extinguisher for the months of January- July 2023.

The transportation document had children who ride the van in the morning checked for the afternoon route. All equipment shall be arranged so as not to obstruct supervision of children.
During a Monitoring Visit conducted on October 6, 2022 ; It was determined based on observation that the red table near the far back gate had chipped paint and rust with exposed sharp edges. It was further determined that the middle section and right side of the PVC piping was not connected. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a Licensing Study conducted on September 27, 2023 ; It was determined based on observation that the following hazards were found on the playground: - The blue and red lemonade stand on the far right side had chipped paint on top of the inside. - The yellow and red merry- go- round cycle had chipped paint and rust. - The wooden seat on the blue and red metal bike was splintered with exposed screws. - There were two blue "Little Tikes" rocking horses missing the rubber grips on the handles. The yellow rocking fish was missing the rubber grips on the handles. - Under a wood covering between the back doors of both classrooms A and B was a blue "Safety 1st" Trike with a crack on the seat.
