During a Licensing Study conducted on August 3, 2022 ; It was determined based on observation that a one-year-old was observed to run out of classroom C into the hallway unattended because the door was not closed, and the child safety gate was not in use to help ensure children are secured in a supervised area. Additionally, a staff member in classroom E stepped out of the classroom into the hallway briefly leaving 5 one-year-old and 2 two-year-old children in the classroom unattended. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a Monitoring Visit conducted on February 9, 2022 ; It was determined based on observation that seven children in classroom B and nine children in classroom C were observed to be asleep on mats that were one inch thick and were not at least two inches thick as required.

During a Monitoring Visit conducted on February 9, 2022 ; It was determined based on observation and staff statements that the mat coverings were stored in a stack with the sheets and blankets touching and were not stored individually as required. Staff stated that the mat coverings are washed once a week on Fridays.

Records of completion of such training shall be maintained, as required by these rules.
