During an Incident Investigation Closure conducted on March 28, 2023 ; It was determined based on an investigation that on February 8, 2023, a staff member grabbed the wrist of a one-year-old child to prevent the child from running during diapering, when the child's right wrist snapped. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During an Incident Investigation Closure conducted on April 17, 2023 ; It was determined based on investigative findings that on March 30, 2023, a center staff member was holding a two-month-old infant and bent down to assist another child in the stroller. The staff member bent over with the infant within their arms and the infant moved to the opposite side causing the staff member to lose their balance and fall. The staff member and the two-month-old infant fell to the floor. The infant's head was secured within the staff member's hands and no serious injuries occurred as a result. The two-month-old infant received a mark on their head from the staff member's ring. Exhibit "A"

During an investigation, it was determined that on February 8, 2023, center staff grabbed a one-year-old child in a manner that caused an injury. The child sustained an injury to the arm after a staff member grabbed the child by the hand and wrist. The child required professional medical attention to treat the injury.
