    During a Licensing Study conducted on November 16, 2023 ; It was determined based upon observation that the following hazards were accessible to children on this date: First Back Left Classroom ‡One pack of Parents Choice wipes that read keep out of reach of children were located inside a child¶s cubby. ‡One plastic grocery bag was located inside a child¶s cubby. ‡Two pairs of adult scissors and one adult stapler were located inside a cubby shared with the children¶s cubbies.

    During a MV POI Follow Up conducted on May 13, 2024 ; It was determined based on observation that the following hazards were accessible to children on this date: Main First Back Left ‡A red Craftsman blower was located on the floor within the children¶s bathroom. Main Middle Back ‡A Staff member¶s black book bag was located on a child¶s table on the left side of the classroom door. ‡Four plastic grocery bags were located inside children¶s cubbies. ‡A broom and dust pan were located next to the trash can. ‡A black cabinet next to the children¶s bathroom door was unlocked with a large black trash bag and a roll of clear trash bags. ‡The children¶s bathroom had a packet of children¶s wipes accessible that read keep out of reach of children. ‡The children¶s bathroom had a plastic grocery bag containing extra plastic grocery bags accessible to children. Main Back Right ‡Two plastic grocery bags were stored within a child¶s cubby. ‡The diaper changing table was unlocked and had plastic grocery bags stored underneath that were accessible to children. Main Second Front Right ‡A plastic grocery bag was stored within a child¶s cubby. ‡A broom and dust pan were accessible to children. ‡A bottle of Ajax Ultra dish soap was located on the left side of the children¶s handwashing sink. ‡One bottle of Purell Professional Surface Disinfectant, three cans of Clorox 4 in One Disinfectant Spray, one can of Clorox Disinfecting Spray and three plastic containers of an unidentified foam soap were observed inside the cabinet of the children's handwashing sink.

    During an Incident Investigation Closure conducted on March 15, 2024 ; It was determined based on a investigation that on December 12, 2023, a two-year-old child sustained head injury when a shelf fell over and hit the child on the head. Center staff failed to ensure that the classroom equipment was properly secured to prevent the equipment from being pushed over by a child. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.
