During a Complaint Closure conducted on November 25, 2024 ; It was determined that on April 14, 2024, at approximately 3:30 p.m., a three-year-old child was observed to be wandering in the center's front parking lot alone for approximately one minute without staff knowledge.

During an Incident Investigation Closure conducted on August 22, 2024 ; It was determined, based on investigative findings, that a two-year-old child was left unattended on the playground for approximately 12 minutes and one second on July 25, 2024. Based on video footage, the two-year-old child was left outside unsupervised from 10:29 a.m. to 10:44 a.m., and staff were unaware that the child was missing until returned to the classroom. Center staff were notified, by staff at a local school that was adjacent to the child care center, that the child was unattended on the playground.

During an Incident Investigation Closure conducted on August 22, 2024 ; It was determined that while on the playground a two-year-old child had direct access to four kiddie pools that were filled appoximately half way full and contained two to three inches of water. The child was observed to enter one of the kiddie pools and remained in the kiddie pool for approximately one minute. Center staff were notified, by staff at a local school that was adjacent to the child care center, that the child was unattended on the playground.
