- Violation
During a Complaint Closure conducted on April 17, 2025 ; It was determined based on an investigation which included video footage, that on March 4, 2025, staff members did not remain alert and failed to actively supervise the children in their care for a period of time while the children were on the playground. Staff were observed to be standing at the gate talking with one another, cleaning up toys in a area that was away from where the children were playing, and waving to individuals in the adjacent parking lot. Exhibit "A"
- Violation
On December 4, 2024, during an investigation, it was found that center staff failed to ensure proper oversight when a two-year-old child was left outside on the playground for 13 minutes after the staff returned indoors. Staff did not follow name-to-face procedures for the transition from playground to building. When the iPad used for these procedures became inoperable, staff relied on hand-counting the children, but some children were missed while entering the classroom. According to staff statements, the class was returning into the classroom and some children were leaving out of the classroom. This caused staff members to have an inaccurate number of children when conducting the class head count. Video footage showed that staff lined the class up on the playground and counted each child, while the two-year-old child was still on the playground. Video footage further showed the child running to catch up with the class as they returned indoors but the door was closed before the child could enter. Staff were unaware of the child’s attempt to enter. The outdoor temperature was 52°F.
