During a Monitoring Visit conducted on February 5, 2026 ; It was determined based on observation that the electrical outlets in Main B/middle did not have protective caps and were accessible to children.

During a Licensing Study conducted on September 18, 2025 ; It was determined based on observation that the following items need repair and/or cleaning: Main Room B: -Two light bulbs were missing from Main Room B/middle in the sitting area. -Two of two light bulb fixtures were missing from the ceiling. -Eight holes were present in the ceiling area above the play kitchen. -Peeling paint was present on the divider on the right side of the room. -Water damage was present on the ceiling. -Vents throughout the classroom required cleaning. Main Room C: -Vents through the room required cleaning. -A filter needed cleaning and fixtures needed to be replaced. -Water damage was present on the ceiling.

During a Monitoring Visit conducted on February 5, 2026 ; It was determined based on observation that the following items need repair and/or cleaning: Main Room B: -Two light bulbs were missing from Main Room B/middle in the sitting area. -Two of two light bulb fixtures were missing from the ceiling in the back area. -Eight holes were present in the ceiling above the play kitchen in the middle area. -Water damage was present on the ceiling causing water stains and protruding ceiling tiles in the middle and back areas. -Vents in the middle and back areas of the classroom required cleaning from dust and rust. Main Room C: -Vents in the back area required cleaning. -A filter needed cleaning and fixtures needed to be replaced above the cubby area. -Water damage was present on the ceiling in the back area.

During a Monitoring Visit conducted on June 23, 2025 ; It was determined based on observation that the right side fence on the main playground was not secured and had sharp edges at the bottom of the chain link. It was further determined that there was a gap that measured 6.5 inches, which posed a potential entrapment hazard. Additionally, there was a 3-inch gap on the fence that was located on the right side of the playground that was open and was a pathway to the street.

During a Licensing Study conducted on September 18, 2025 ; It was determined based on observation that the following areas presented a potential fencing hazard: -A chain link gate located on the right side of the playground had a gap that measured 4.5 inches, which caused a potential entrapment hazard. -The chain link fence was detached from the top pole at the left side of the playground. -Chain links were flipped inward causing sharp edges to be exposed. -A broken plank was present on the wooden fence on the playground causing splintering wood.

The hole had sharp and rough edges and chipped paint. -The red merry-go-round was broken and leaning on the sides. -The child-sized Connect Four game was cracked. -The toy storage cart was missing a wheel. -The yellow train was missing a steering wheel. -The red spinning wheels had standing water and were rusty. -The green picnic tables had chipped paint.
