    591-1-1-.26(8) · Rule 591-1-1-.26(8) requires climbing and swinging equipment to have a resilient surface beneath the equipment and the fall zone from such equipment must be adequately maintained by the Center to assure continuing resiliency.

    During an Initial Licensing Study conducted on March 3, 2023 ; It was determined based on measurements that the resilient surface beneath the tall red slide had less than one inch of resilient surface at the end of the slide and less than two inches in the fall zones of the slide, when three inches was required.

    591-1-1-.30(1)(a)2 · Rule 591-1-1-.30(1)(a)2 requires that a crib mattress is firm, tight-fitting without gaps, at least 2 inches thick and covered with a waterproof, washable material. Before a change of occupant, each mattress shall be cleaned with a disinfectant.

    During an Initial Licensing Study conducted on March 3, 2023 ; It was determined based on observation and measurements taken that the crib mattresses were too small for the cribs. There were one inch to two inch gaps between the mattresses and the side of the crib in six out of six cribs.
