During an Incident Investigation Closure conducted on December 14, 2023 ; It was determined based upon investigative findings that on November 29, 2023, center staff did not notify the parents of a child¶s injury; when a two-year-old child fell from a diaper changing table.

During an Incident Investigation Closure conducted on April 12, 2024 ; It was determined based on investigative findings that center staff did not immediately notify the parents of a one-year-old child when the child fell and hit their head in the classroom.

Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. "Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers and Students-in-Training. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs. Staff shall be attentive and participating with all children during mealtimes and shall be seated within an arm¶s length away from children thirty- six (36) months of age and younger.
During an Incident Investigation/Monitoring Visit conducted on March 4, 2024 ; It was determined based upon observation that the staff member in Classroom G left one child unsupervised in the classroom while the staff member went to use the restroom inside the classroom for approximately one minute.

During an Incident Investigation Closure conducted on March 13, 2024 ; It was determined based upon investigative findings that center staff failed to supervise a three-year-old child when the child was left unattended on the toddler playground for less than one minute when the staff returned with the class to the classroom after outside play.

When storage is available and used for the storage of cots and mats that allows the cots, mats and any bedding to be stored without touching any other cots, mats or bedding, the bedding may be left on the cot or mat. When such storage is not available for the cots and mats, each child¶s bedding shall be kept separate from other children¶s bedding and stored in containers marked for individual use, such as, but not limited to, bins, cubbies, or bags.
During a Licensing Study conducted on November 17, 2022 ; It was determined, based on observation, that children's blankets were observed to be stored touching other children's blankets on this date in Rooms G and H.

During a Licensing Study conducted on November 17, 2022 ; It was determined, based on observation, that the following potential hazards were observed present on this date: Room E ±A broom, dustpan, and microfiber mop were observed accessible to children in the classroom. A bottle of CeraVe Daily Moisturizing Lotion was observed accessible to children in a child¶s cubby. Plastic trash bags were observed accessible to children near the stacked chairs. Room F ±A broom and dustpan were observed accessible to children in the classroom. Room G ±A plastic bag was observed hanging from the hooks under the children¶s cubbies on this date. Room H ±Plastic bags were observed accessible to children on the top of a low shelf and in two children¶s cubbies on this date. Adult scissors were observed accessible to children on a low shelf on this date. A box of gloves was observed accessible to children on top of a shelf in the bathroom on this date.

During a Monitoring Visit conducted on January 31, 2023 ; It was determined, based on observation, that the following potential hazards were observed present on this date: Right Hallway: The laundry room was unlocked and contained several bottles of cleaning products. Classroom D : One broom and mop were accessible in the children's bathroom, the cabinet door under the sink was disconnected at the hinges and had two exposed screws. Classroom G: One box of latex gloves and one bag of children's wipes were accessible to children on a low shelf in the bathroom, and one drawer by the sink was unlocked containing a stapler, crazy glue and two stapler removers. Classroom H: An unlocked cabinet by the sink contained one bottle of snow spray, one bottle of lysol disinfectant, and one plastic bag with Christmas lights and wires.

During a Licensing Study conducted on November 17, 2022 ; It was determined, based on observation, that Room B was observed to have a ratio of 1:8 when a ratio of 2:8 was required due to the classroom consisting of four infants and four one-year-old children on this date. It was further determined that Room H was observed to have a ratio of 1:15 when a ratio of 2:15 was required due to the classroom consisting of nine two-year-old children and six three-year-old children on this date.
