591-1-1-.25(3) · Rule 591-1-1-.25(3) requires the Center and surrounding premises to be kept clean, free of debris and in $499.00 good repair. Hygienic measures such as, but not limited to, screened windows and proper waste disposal procedures shall be utilized to minimize the presence of rodents, flies, roaches, and other vermin at the Center.

During an investigation, it was determined that on September 21, 2021, a three-year-old child got up from a rug where they were seated for an activity, went over to an area of bare floor that had recently been cleaned by a staff member, slipped, and fell. The child sustained an injury that required professional medical attention. The recently cleaned area of floor was not blocked off in any manner to have prevented access by children. It was revealed that the area of floor was slippery when touched by a staff member after the incident had occurred. .
