During an investigation, it was determined that on April 17, 2026, at approximately 9:50 a.m., center staff did not intervene when a 4-year-old child repeatedly spun in place inside a classroom near a countertop. At the time of the incident, two staff members were supervising 15 children ranging in age from 3 to 4-years-old. The child leaned over and struck their head, resulting in an injury. First aid was administered, management was notified, and the child’s parent was contacted. Professional medical attention was obtained as a result of the injury.

During an investigation, it was determined that on June 30, 2021, a two-year-old child sustained a laceration after tripping and falling on classroom equipment. One staff member supervised 3 two-year-old children. The teacher instructed the children to walk to their cubbies to prepare to go home. While going to their cubby, the two-year-old child tripped and fell, hitting his forehead on a stack of chairs that had been placed near a shelf. The equipment was not placed in a way to minimize the danger of accident and collision. The teacher heard the two-year-old child begin to cry and went to attend to the child. The teacher observed a cut on the child’s forehead and administered first aid, then reported the incident to administration and called the child’s mother. The child received professional medical attention and received two stitches to the forehead.
