During a Monitoring Visit conducted on September 23, 2024 ; It was determined based on observation that the following hazards were observed: F - A hanging phone cord on the right side of the classroom accessible to children in care. - A plastic bag on top of the cubby located under the fire extinguisher accessible to children in care. D/E - Diaper wipes on top of the shelf on the right side of the classroom accessible to children in care. - A hanging radio cord on the left and right side of the classroom accessible to children in care. G - A loose cable cord in the first restroom with a protruding nail accessible to children in care. H -Clorox wipes on the sink counter accessible to children in care. - Lysol wipes, sanitizer, and diaper wipes in a clear container on top of the manipulative shelf accessible to children in care. K - A plastic bag on top of the table accessible to children in care. - A plunger in the restroom accessible to children in care.

During a Licensing Study conducted on February 18, 2025 ; It was determined based on observation that the following hazards were observed: Classroom A - Rust was observed on the floor vents throughout the classroom. Classroom C - Plastic grocery bags hanging from the cubbies accessible to children in care. - A hanging cord on the left side of the classroom accessible to children in care. Classroom D/E - A hanging TV cord on the back left side of the classroom accessible to children in care. - A hanging Alexa cord on the front right side of the classroom accessible to children in care. Classroom G - A protruding screw hanging on an electrical cord at the entrance of the second restroom.

591-1-1-.26(4) · Rule 591-1-1-.26(4) requires that playgrounds be protected from traffic or other hazards by a (4) four foot high fence or other barrier approved by this Department. Fencing material shall not present a hazard to children and shall be maintained so as to prevent children from leaving the playground area by any means other than through an approved access route. Fence gates shall be kept closed except when persons are entering or exiting the area.
During a Monitoring Visit conducted on June 18, 2024 ; It was determined based upon observation that the bottom of the fencing was not secured creating a six inch gap on the left side of the playground. It was further determined that the bottom of the fencing was not secured creating a ten inch gap on the back side of the playground causing a potential entrapment hazard. It was also determined that there were five out of five bolts protruding over two threads on each side of the gate entrance door on the back side of the playground.
