During an investigation, it was determined that on July 23, 2025, at approximately 4:13 p.m., Center Staff failed to provide watchful oversight when they closed a door and a 1-year-old child's right hand was caught in the hinges. According to staff statements, three staff and 15, 1-year-old children returned from the playground into the classroom. One staff member was positioned at the back door that led from the playground into the classroom, another staff member was gathering children to be seated on the classroom floor, and the third staff member was conducting a face-to-name count of the children. The 1-year-old child placed their fingers in the door jamb near the hinges of door and the child’s fingers were closed in the door by the staff member. The staff member responded immediately when they heard the child crying. The staff member freed the 1-year-old child’s fingers from the door and staff provided first aid. Staff did not ensure the doorway was clear prior to closing the door. The 1-year-old child sustained a laceration on the middle and pinky fingers of their right hand. Professional medical attention was obtained and stitches were required to close the wounds.

During an investigation, it was determined that on July 12, 2023, at approximately 12:21 p.m., a five-year-old child sustained an injury to their hip when they were forced down on their cot by a staff member. The five-year-old child was running and jumping around the classroom, playing in the water fountain, and was not listening to instructions to lie down on their cot. Video footage showed the staff member forcefully putting the five-year-old child down onto their cot. Professional medical attention was obtained, and the five-year-old child sustained a bruise to their hip. The staff member was fired by the center and arrested. 2. Rule 591-1-1-.31(13) prohibits Center Staff from committing any criminal act, as defined under Georgia law, in the presence of any child enrolled in the Center. Center staff shall comply with all applicable laws and regulations. During an investigation, it was determined that on July 12, 2023, a five-year-old child sustained an injury to their hip when they were forced down on their cot by a staff member. The staff member was terminated on July 14, 2023, and arrested on July 28, 2023.
