    During an incident investigation, it was determined that on November 19, 2021, center staff failed to ensure that all equipment in the classroom was safe when a four-year-old child stood up from the carpeted area and hit their head on the sharp edge of the dry erase board. It was reported that the child sustained a cut near their eye that required professional medical attention and six stitches to close the wound.
