During a Complaint Closure conducted on August 18, 2025 ; It was determined based on investigative findings that the food menu provided for the month of July 2025 did not list what was served for breakfast as all breakfast components were not listed on the menu. It was also observed the month of July 2025 was posted on the parent board with the month of July written out but the dates listed on the food menu were not the same dates for the month of July.

During a Complaint Investigation & Monitoring Visit conducted on April 22, 2026 ; It was determined based on observation that the following hazards were observed: Classroom AL - TB Fresh deodorizer labeled keep out or reach of children was on the edge of the counter and was accessible to children. - A charging cord and extension cord were laying on the floor and were accessible to children. - Lysol disinfectant spray labeled keep out or reach of children was on the edge of the counter and was accessible to children. Classroom CL - A bathroom brush scrubber was stored in the bathroom and was accessible to children. - The white and pink electrical cords were plugged into the wall; the cords were hanging and accessible to the children. - A mop and bucket were located outside of the door to enter the playground and were accessible to the children.

Staff were unaware the two children had exited the classroom alone. Staff stated they had no knowledge of the amount of time the children were unsupervised and the children had no injuries.

It was also observed that the black gate that surrounds the air conditioning unit had torn wires on the top right side.
