During a Licensing Study conducted on September 30, 2025 ; It was determined based on observation that the step in the 2-year-old room was observed to have wood that is rotting. Peeling paint was observed in the 2-year-old room. The 3-year-old girls restroom was observed to have a light fixture in need of repair.

During a Complaint Closure conducted on March 8, 2024 ; It was determined based on investigative findings that the center was not kept clean when several dead roaches were observed in a kitchen cabinet that contained empty storage food containers.

During a Complaint Closure conducted on April 2, 2024 ; It was determined based on investigative findings that on March 1, 2024, the kitchen was observed to have dead bugs laying on shelves throughout the cabinets where dishes were stored. It was further determined that rodent droppings were observed next to a bag of sugar, cigarette filter tip was observed inside of a bowl, and the kitchen stove was dirty with dried up food debris with a pot filled with dirty water.

During a Complaint Investigation & Licensing Study conducted on February 29, 2024 ; It was determined based on observation that chipped paint was observed throughout the walls and the green, orange, and yellow bookshelves located in classroom 3R Front. A ceiling tile located in classroom 5R Right side was not secured to the ceiling and drooping down. The mirror hanging above the first-left sink located in classroom 4R Front was observed to contain chipped paint at the base of the mirror. Protruding screws were further observed on the sides of the toilets located in classroom 4R Front. It was further observed that the walls on the right side of the building at the exit doors of classroom 4R Front and classroom 5R Right Side leading to playground B contained chipped paint.

During a Complaint Investigation & Monitoring Visit conducted on May 16, 2024 ; It was determined based on observation that a live roach was observed in the lobby and in the kitchen pantry to the right side of the refrigerator. Dead roaches were observed in the kitchen pantry and alongside the 4R classroom.

During a LS POI Follow Up conducted on November 4, 2024 ; It was determined based on observation that a cockroach was observed on the floor of the center's lobby area next to the purple and white cabinet storage area. It was further determined based on observation that a live roach was observed crawling inside the double sink located on the back-left side of the kitchen. Dead gnats were observed throughout the refrigerator located on the front-right side of classroom 5R Right Side. While the refrigerator was observed to be on and running, no food items were stored in the refrigerator. Classroom 5R Right Side was observed to be vacant during the visit.

During a Complaint Investigation & Licensing Study conducted on February 29, 2024 ; It was determined based on observation that the two white riding horses contained chipped paint located on the back-right side of playground A and the second blue swing chain was observed to be peeling on the front-left side of playground B.

During a LS POI Follow Up conducted on November 4, 2024 ; It was determined based on observation that the following items were in need of repair: - Playground A - The red, green, yellow, and blue sliding structure was broken into three pieces located on the front-right side of the playground area. The green cover to the sand box contained cracks creating a potential pinching hazard located on the back-right side of the playground area. - Playground B - The red, yellow, and green wooden ladder contained peeling tape exposing the rust on the bars. No children were observed on the playground area during the visit due to inclement weather.

During a Complaint Investigation Follow Up conducted on March 22, 2024 ; It was determined based on observation that the following hazardous items were observed on the playground areas: - Playground A - Pine cones and leaves were observed throughout the grounds on the back side of the playground area. - Playground B - Pine cones, leaves, and grass were observed throughout the grounds on the back side of the playground area. A protruding green stick was observed on the back left side of the playground measuring approximately two feet and four inches in height.

During a Complaint Closure conducted on April 2, 2024 ; It was determined based on investigative findings that dead tree limbs were observed hanging over the playground area posing a risk of falling inside of the playground area where children play. A broken tree branch hanging from a tree on the opposite side of the fence surrounding playground B and low hanging tree branches on the back-left side of playground B. Further walk-through of the playground areas revealed that tree branches were observed leaning against the fence located on the back-left side of playground B and low hanging tree branches were observed on the back-left side of playground B. Two trees were also observed leaning on the opposite side of the fence towards playground B posing a potential risk of falling onto the playground area. A low hanging tree branch was observed on the opposite side of the fence surrounding playground A.

During a LS POI Follow Up conducted on November 4, 2024 ; It was determined based on observation that the following hazardous items were observed on the playground areas: - Playground A - A protruding stick was observed on the back middle side of the playground next to the yellow school bus. Exposed tree roots were observed on the front-left entrance and on the left side of the yellow school bus located on the back middle side of the playground. - Playground C - Two spider webs, one bird nest, and a bee hive were observed underneath the covered porch located on playground C. The exposed tree roots and protruding stick were observed to create a potential tripping hazard to the children. The bee hive, spider webs, and bird nest appeared to be inactive during the visit. No children were observed to be playing on the playground areas during the visit due to inclement weather.

During a Complaint Investigation & Licensing Study conducted on February 29, 2024 ; It was determined based on observation that the brown, red, and green slides did not have any resilient surface surrounding the fall zones of both slides located on playground C. Both slides were approximately four feet in height and required approximately three inches of resilient surface. The consultant further observed loose green turf throughout the grounds of playground C.

During a LS POI Follow Up conducted on November 4, 2024 ; It was determined based on observation that the black mats used for resilient surface underneath the yellow, red, and blue swings were not flush with the sand creating a potential tripping hazard. It was further determined that the green, red, yellow, white, and brown slide structures did not contain any resilient surface surrounding the fall zones of both slide structures located on playground C. The fall zones surrounding the slide structures were observed to be sitting on a green layer of turf with no cushion around the fall zones. Exhibit "A"

Also, the path from the classroom to Playground B from the 4R back classroom exit was observed to have a border leaning and in need of repair. There was a loose board with a hole in the border from the path to and from the classroom upon entrance at the gate. Exposed roots were observed from the path to the playground.
