    During a Complaint Investigation & Monitoring Visit conducted on October 4, 2023 ; It was determined based on observation that vehicle tag #PNM 1308 was not in good repair as the bench seats had torn fabric and exposed foam on this date.

Per director, the center has not provided any transportation services to the enrolled children since November 2023.
