591-1-1-.03(9) · Rule 591-1-1-.03(9) requires staff shall not engage in or allow children or other adults to engage in activities that could be detrimental to a child’s health or well-being, such as but not limited to, horse play, rough play, wrestling, and picking up a child in a manner that could cause injury.

During an investigation, it was determined that on January 5, 2026, at approximately 5:30 p.m., a 3-year-old child was allowed to run inside the classroom and fell, striking the left side of their face on a table. At the time of the incident, there was one staff member, and four 3-year-old children present in the classroom. The children were sitting at the table around 5:30 p.m. engaging in table activities, when two children were running around the classroom tables. Staff instructed the children to use their walking feet in the classroom as a form of redirecting the children’s actions. According to video footage, after the staff member intervened, one child stopped running, however, the other child continued to run, fell, and hit their eyebrow on the corner of the table. Center management and the child’s parent were notified of the incident. Staff failed to physically intervene when the 3-year-old child continued to run around the table in an unsafe manner, which resulted in the 3-year-old child being injured. The child sustained eyebrow laceration to the left side and a mild head injury. Professional medical attention was obtained, and the child received four sutures.
