591-1-1-.25(3) · Rule 591-1-1-.25(3) requires the Center and surrounding premises to be kept clean, free of debris and in good repair. Hygienic measures such as, but not limited to, screened windows and proper waste disposal procedures shall be utilized to minimize the presence of rodents, flies, roaches and other vermin at the Center.

During a Monitoring Visit conducted on July 11, 2023 ; It was determined based on observation that the paper towel dispenser was rusted in the bathroom in classrooms C, E, and G. Additionally, the faucet in the boys' bathroom in classroom G had rust.

During a Licensing Study conducted on February 5, 2024 ; It was determined based on observation that in classroom E the drywall on the orange wall near the art area and back door was chipped. The drywall on the orange wall on the right side of the window was chipped with an exposed screw. In classroom C the toilet was missing the tank top. Additionally, the classrooms near the infant to two-year-old's playground had two black screens on the outside of the building that were torn.

During a POI Follow Up conducted on April 29, 2024 ; It was determined based on observation that in classroom E the drywall on the orange wall under the back window was chipped and the walls had peeling paint throughout. In classroom F the drywall on the pink wall on the back window sill was chipped and the walls had peeling paint throughout.

During a Licensing Study conducted on February 5, 2024 ; It was determined based on observation that the sliding door to the vehicle with license plate number RAR 4235 did not open, which could impede the children¶s access or egress from the vehicle. Additionally, the first row seat was torn.

591-1-1-.36(4)(b) · Rule 591-1-1-.36(4)(b) requires that the interior of a transportation vehicle be clean, in safe repair and free of hazardous items, objects and/or other non-essential items which could cause injury.

During a POI Follow Up conducted on April 29, 2024 ; It was determined based on observation that the sliding door to the vehicle with license plate number RAR 4235 did not open from the outside after it was opened and closed. The sliding door did not open or close with ease from the inside which posed a hazard because it could impede the children¶s access or egress from the vehicle.
