    During a Licensing Study conducted on June 4, 2024 ; It was determined based on observation that diapers in the original plastic packaging and wipes in the original plastic packaging were observed to be in children bags and cubbies in classroom B.

    During a Licensing Study conducted on October 31, 2024 ; It was determined based on observation that the following hazards were accessible to children: Classroom A - A white trash bag was sitting inside of a white crib. Classroom B - Three grocery bags inside the cubbies. - One box of vinyl gloves and one box of latex gloves were on top of the changing table. - "Boudreaux's" butt paste was on top of the changing table. Classroom C - Pull-ups in the original plastic packaging inside a cubby. - Wipes in the original plastic packaging labeled "keep out of reach of children" on top of the cubbies. - One blue grocery bag on top of the cubbies. Bathroom - There was an air freshener sitting on top of the soap tray.

    591-1-1-.14(1) · Rule 591-1-1-.14(1) requires the Center Director and, at any given time, at least fifty percent (50%) of the caregiver Staff to successfully complete a biennial training program in cardiopulmonary resuscitation (CPR) and a triennial training program in first aid. The annual ten (10) clock hours of training shall be chosen from the following fields: child development, including discipline, guidance, nutrition, injury control and safety; health, including sanitation, disease control, cleanliness, detection and disposition of illness; child abuse and neglect, including identification and reporting, and meeting the needs of abused and/or neglected children; and business related topics, including parental communication, recordkeeping, etc.; provided however that such business related training shall be limited to no more than two (2) of the required ten (10) clock hours of training. Records of completion of such training shall be maintained, as required by these rules. During a Licensing Study conducted on June 4, 2024 ; It was determined based on a review of records that staff #2 and staff #6 did not have evidence of 10 hours of annual training for 2023. 4. B. Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. "Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers and Students-in-Training.     During a Licensing Study conducted on June 4, 2024 ; It was determined based on observation that one staff present with eleven children had nine children that were not in sight due to the shape of classroom C which resulted in the supervision plan not being followed.

    During a Licensing Study conducted on October 31, 2024 ; It was determined based on observation that one staff present with five children walked four children from classroom C over to classroom B leaving one child in classroom C alone. The staff member stood at the door of classroom B calling for the child to walk across the hall from classroom C to classroom B. Additionally, a staff member asked the director to step into classroom C while she left the classroom. The director came into the classroom but then stepped back into the office leaving the child unsupervised. Exhibit "A"
