20-4: During the inspection, Licensing Specialist observed that there was one infant sleeping, swaddled, with their arms restrained. During the inspection, staff took the infant out of the swaddle. During the inspection, Licensing Specialist observed the package of pancakes in the freezer opened and not dated. The director disposed of the pancakes putting this back into compliance. 47-1: During the inspection, Licensing Specialist observed the recorded video from the 8/9/2024 incident. Licensing Specialist observed the teacher walk over the child and pull the child up by their hand/wrist. The child drops her weight down while pulling away from the teacher. Licensing Specialist observed that childs flex outward as the teacher continues to hold the childs hand/wrist. The teacher led the child to the diaper changing table. When the kids were sitting down for snack, the child stated that her arm hurt. The teacher gave the child some ice and the office staff called the parents. The parents came and took the child to the doctor. The doctor stated that the child had Nursemaid Elbow due to the teacher pulling the childs hand/wrist. The child was moved into a different classroom with a new teacher and an incident report was created for the incident. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 8.2, C: Child care personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. On 3/25/2024, Licensing Specialist watched the playground cameras from 4/12/2024 and observed three classrooms outside on the largest playground with 3 teachers and 27 children, two-year-olds, three year olds and four year olds. From 10:25:40 to 10:29:40 (camera time stamp) child 1 and child 2 are playing together by throwing their shoes. At 10:28:50 child 1 threw a shoe and accidently hit teacher #1 in the head. At 10:32:25 teacher #1 took off her shoe and threw it at child #2 and hit him in the head (the shoe was not thrown hard). Child #2 grabbed the back of their head. The director advised that teacher #1 was terminated and reported the incident to Licensing and the Child Abuse and Neglect Hotline. The is being marked corrected at the time of the inspection. 1:4 infant 2:12 one year olds 1:11 two year olds 1:15 three year olds and four year olds 1:12 four year olds and five year olds 1:11 four year olds and five year olds 3-01: During the inspection, Licensing Specialist reviewed the cameras of the playground on 3/8/2024 and observed that at 10:00 am there were two classrooms outside on the playground with 2 teachers and 19 children (18 three year olds and 1 four year old). At 10:04:10 am EG goes inside leaving MG out of ratio outside with 1 teacher and 19 three year olds and a four year old (18 three year olds and 1 four year old). At 10:07:29 EG came back outside, being the outdoor classes back into compliance. At 10:08:20 MG went inside leaving EG out of ratio outside with 1 teacher and 19 three year olds and a four year old (18 three year olds and 1 four year old). At 10:10:05 the Blue Room came outside with 10 two year olds and 1 teacher, EO. There is now a ratio of 2 teachers and 29 children (10 two year olds, 18 three year olds and 1 four year old). The majority of the children are three year olds making the ratio 1:15. This bring the outdoor classrooms into compliance. At 10:24:20 EO went inside. At 10:25:21 one child and MG went inside leaving EG out of ratio outside with 1 teacher and 28 children (10 two year olds, 17 three year olds and 1 four year old). At 10:27:48 MG came back outside with the one child. There is now a ratio of 2 teachers and 29 children (10 two year olds, 18 three year olds and 1 four year old). The majority of the children are three year olds making the ratio 1:15. At 10:29:08 EO came back outside. This is being marked resolved. During the inspection, Licensing Specialist provided technical assistance, STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4.
