At the time of the inspection, information was obtained that revealed a teacher (named on supplemental) stated that the child could have gotten hurt by several different instances; she recalled once by climbing a 4 foot fence and falling off during climbing the fence; also, he was in the bathroom with the door closed; she heard a thump and when he came out, he said that he fell, and he hurt, his back and butt area. He plays in the toilet with the water regularly which is why they usually go to the bathroom with this student but on this particular day she did not and the door was closed. The lack of supervision could have caused the childs injuries. The teacher was aware by witnessing a child fall and remembered the child did mention that he was hurting throughout the day and did not check to ensure injuries did not need aid, nor did she document them on incident reports. The child according to the teacher, fell from jumping off the fence into the mulch, off a tricycle on to the concrete slab, backwards falling onto his bottom bracing his fall from a step stool leading to the water fountain, and in the bathroom coming out to tell her that his butt was hurting. The noncompliance was resolved because the child was removed from the facility and no longer in the direct supervision of the teacher

At the time of the reinspection, when the counselor arrived to commence the inspection, none of the teachers were in their respective classrooms during. One teacher stated she went to answer the door because she thought I was her food delivery (infant) and another teacher stated she came to answer the door (3 year old). Two year old teacher was out in the hallway retrieving a child that was sleeping in the hallway with their sleep mat as the counselor did her walk through.
