Per teacher (CM), She was in the restroom when the group of 1y/o to 3y/o came into the classroom from the playground. She stated that the two other teachers in the group were aware that she was in the restroom. When asked if anyone covered her while she stepped out of the classroom, she said no. During her time away from the group, there were a total of 13 children of mixed ages 1y/o to 3y/o with 2 staff members (not including the child that was left on the play area, CR)

A complaint allegation was received on 2/24/2023 alleging that On 2/23/23, Child named CR had left her preschool and was found on the side of a busy road across the street from the school. Chloe was in a group of 14 children as they were going inside from the playground. She had wandered away to a secondary part of the playground that is connected to the first part by a gate that does not close properly. The rest of the class and the three supervising adults went inside at 10:53AM. Chloe slipped through another gate at 10:55AM that was not closed properly by the lawn maintenance workers. Chloe walked out towards the busy street and was found by a passerby at 10:56AM. The passerby returned Chloe to the school. No one had noticed Chloe was gone until the passerby brought her back. A complaint inspection was conducted on 2/24/2023 and statements from the Director/owner (KC) and assigned teachers (PM and CM) were received. Statement from third teacher (ELR) assigned to the group was not received a she was not present at the time of the inspection. Video footage of the incident was also reviewed. The following was observed from the video footage; Child slipped through a gap in a double gate located at the large playground at 10:55AM. The video footage recorded the time as 9:55AM but per the owner, the camera recording system has the wrong time as it is one hour behind the real time. The child walked down the turnaround parking lot, toward the road. Once she reached the end of the lot, she walked to the right side (west) on 26th street. She walked off the sidewalk and toward the road, stopping on the white roadside parking lines. At this point, she walked back unto the paved side walk. A truck heading east pulled over, crossed the street and picked up the child. He went back to his truck to retrieve something with the child in his arms and proceeded back to the child care building. The following statements were received: Per the Owner/Director (KC), The children were in the small courtyard transitioning into the classroom form the playground. A teacher takes 1 or two children at a time to use the restroom that is in the classroom. The two staff that remained in the courtyard with the other children did their count and thought that CR was in the restroom with teacher number 3. The entered the building with the group and once in the room they noticed CR wasn't there. There were a total of 3 teachers and 14 children, including CR. As the teachers begin to realize the child was missing, someone knocked on the main entrance door. When the office staff opened the door, a man holding a child asked "is this yours?" unaware of who the child was, the office person brought the Director up to the door who quickly realized that the child was CR. The child was retrieved and returned to her assigned group. Per teacher (PM), she was in charge of restroom routine. She took children to the restroom. The staff came in with the rest of the children shortly after. Ms. ELR realized the child was not with the group and returned to look for her. At this point, the director, Ms. KC had already had the child in her arms. Per teacher (CM), She was in the restroom when they came back in from the playground. The two teachers were aware that she was in the restroom. When asked if anyone covered her while she stepped out of the classroom and she said no. During her time away from the group, there were a total of 13 children with 2 staff members (not including the child, CR). This complaint is deemed SUBSTANTIATED as the child gained access out of the fenced in play area
