As evidenced by review of documentation provided, the following occurred: -During a field trip on 06/25/2025, provider utilized a 15-passenger van, leased from Enterprise. The vehicle did not have a child safety alarm approved by the Department, as verified by Enterprise representative. Vehicle (VAN) VIN 84357 (Leased from Enterprise) It is unclear if vehicle rented through Enterprise met additional vehicle requirements including VEHICLE SIGNAGE, FIRST AID KIT, FIRE EXTINGUISHER, OR PROOF OF VEHICLE INSPECTION. The following technical assistance is provided: CCF Handbook, Section 2.5.4 and (F) "For the purpose of this section, vehicles refer to those owned, operated or regularly used by the child care program, and vehicles that provide transportation through a contract or agreement with an outside entity. Parents/guardians personal vehicles used for transporting during field trips are excluded from meeting the requirements of this sub-section. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 2. List of approved alarm manufacturers may be found on the Departments website at www.myflfamilies.com/childcare . 3. Alarms must be installed and maintained according to the manufacturers recommendations. 4. Alarm systems installed according to the manufacturers instructions prior to October 1, 2021, will be deemed to satisfy this requirement if they meet the criteria set forth in 2.5.4.F.1. above. Under this allowance, the existing alarm system must always be properly maintained in working order. Should at any time the alarm system require replacement, the new system must be selected from the Departments approved list referenced in 2.5.4 F.2. above. Section 7-45(b), Ordinance "Vehicles must state the name of the child care facility on each side panel in six-to-eight inch letters. The back panel must have the wording "Caution Transporting Children" in a minimum of four-inch letters. The facility's license number also must be printed on the side and back panels of the vehicle." A complaint allegation was received on 4/3/2023 alleging that a child (SW) was sexually assaulted at the preschool. Child alleged another child(L) told SW to go under the desk to play, hide and seek where He unbuttoned her skirt and pulled it down. Child alleged He touched her on the front and on the back of her pantie near her private area. Parent stated she walked into Aftercare, Saw her daughter crying with her pants down. A complaint inspection was conducted on 4/6/2023 and a statement from the director (CW) was received for compliance. Per the director, the incident occurred after hours. The mother was late at picking up her two children so the director took the two girls into her office to wait for their mom. The director was working on something on her computer when the boy (L) came in the room and asked her if he can play hide and seek with the girls. The director said yes and they started playing. Shortly after, they were playing under the table and the mother of the girls walked in. The girl's skort was not down, it was on but it looked crooked/ uneven. When asked what happened to her skirt, she said the boy (L) pulled her skort down and touched her private area over her underwear. Per the Director, the boy (L) was questioned by the director and school head of board members and the child admitted to having touched the child's bottom. This complaint is substantiated as there was inadequate supervision of the children at the time the incident took place
