33-03 · Technical assistance: Observed The facility documented bites incidents separately from other incidents. The facility had another complete form to complete an incident report. According to the director, the bite is documented separately to comply with ELC requirements. At the time of the inspection, the bite incident form was updated by the director to document the time, additional description of the incident, and action taken by whom. The director was told it is important to have the parent sign and date the report to show it was provided to the parent the same day of the incident

Incident report was completed for each child involved. However, the incident report was missing the following: the time of the incident, the description of the event, action taken by whom. - According to the incident reports, the incident occurred 8/6/2025, however, it was unclear at what time. - The description of the incident was just "child DW bit another child today". However, it was missing the details about a fallen tooth resulting from the bite. - The incident report stated child DW bit child KB, however, it was not mentioned how the staff (s) responded to the incident. 45-01 · Bottles Capped and Labeled (Article XII.G)

At the time of the inspection, the inspector observed a child in the one year old classroom with a bottle beside her as she lay on her mat. The bottle was observed with milk and without a label. Technical support was given to label all bottles. This standard was resolved as the bottle was labeled by the facility's director
