During the time of the inspection, the Licensing Specialist observed that in the indoor play area there was a brown chair that was damaged. The left arm of the chair was ripped, exposing the foam of the chair. Tape was previously placed over the damaged area. That tape was torn, and there are strings hanging from the torn tape. This standard is non-compliant, and the provider is being until 3/14/2025 to back into compliance. During the inspection, the Licensing Specialist provided technical assistance. During the inspection, the Licensing Specialist observed that there were knives in an unlocked drawer in the kitchen and a set of knives on the counter accessible to children. This is being marked completed at the time of the inspection in the provider removed the knives from the counter, and the provider's husband placed a lock on the drawer after the children had already left for the day. During the time of the inspection, the Licensing Specialist observed that the beige house with a green roof was broken at the top. Pink Little Tikes Car has a broken plate that is off white in the front of the car, it has sharp edges. The water drain has a clamp wrapped around the drain; the edge of the clamp is sticking out exposing a sharp edge. This standard was brought back into compliance at the time of the inspection in that the Owner removed the beige house with the green roof, and the pink car. The Owner tape the water drain's clamp down so that it was not sticking out with the sharp edge exposed. During the inspection, the Licensing Specialist provided technical assistance. During the inspection, the Licensing Specialist observed a diaper changing mat that did not have an impermeable surface. This standard was brought back into compliance at the time of the inspection in that the Owner replaced the ripped mat with a new one. 31-02 · Immunization Records FDCH/LFCCH Handbook
During the inspection, the Owner advised the Licensing Specialist that there are currently 5 children enrolled. During the inspection, the Licensing Specialist monitored 5 of the 5 childrens records. During the inspection, the Licensing Specialist observed that one of the enrolled children's immunization records were expired. The director was given until March 14, 2025, to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance: FDCH/LFCCH Handbook, Section 8.1 A: Health/Immunization Records: The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C-22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations should provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations

• While the CCR was inspecting the outdoor area the CCR noticed on the little Tykes climber/slide on the steps there is a crack where the children could put their fingers into. 7.6 Outdoor Time, Fencing and Play Area Requirements A.
