During the abbreviated inspection two licensing specialist supervisors observed a spray bottle that contained blue liquid (labeled 844875015345 32 OZ spray bottle SS32 spray), located in the kitchen behind an unlocked door, on a rolling cart accessible to children in care. When the licensing specialist brought it to the attention of the owner, he stated that it might be Fabuloso. Photos and video were taken of the kitchen door, kitchen, and the cart that contained the spray bottle with the blue liquid. This was resolved at the time of inspection when the owner took the spray bottle and placed it in a cabinet not accessible to the children in care. The owner also ordered a lock for the kitchen door

10-15 · Field Trip Permission
2024, in the afternoon between 4:00 pm and 4:30 pm, Suite 250 were outside with two teachers and 11 children. Teacher 1 stated that the children are two years old. Teacher 1. Teacher 1 stated that teacher 2 went inside to get water with two children. While teacher 2 was inside, the drain cover must have been taken off, but she did not see how the children got the cover off. Teacher 1 stated that one of the teachers from the infant classroom opened their outside door and told teacher 1 to get the child out of the drain. Teacher stated that the childs mom was in the infant classroom as a teacher. Teacher 1 stated that she does not know how he got in the drain so fast. Teacher 1 stated that the child was not hurt or crying, he was playing and laughing. Teacher 1 stated that the child had both of his legs inside the drain up until his waist. Teacher 1 stated that she had only turned around for not even 30 seconds and the child got in the drain. Teacher 1 stated that the children in the past have messed with the drain before. The children started messing with it when the facility added turf, however this is the first time that a child got inside the drain. The director stated that the drain cover has been temporarily secured until a permanent solution can be found. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.4 A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group

During the inspection, the director advised that currently there are three transportation vehicles. First vehicle is a Chevy Express (115355) that was last inspected on 06/25/23 and is insured through Church Mutual Insurance Company with an effective date of 05/25/2024. The second vehicle is a Chevy Express (150442) that was last inspected on 06/20/2024 and is insured through Church Mutual Insurance Company with an effective date of 05/25/2024. The third vehicle is a Ford Super Duty (882042) and did not have an updated vehicle inspection and is insured through Church Mutual Insurance Company with effective date of 05/25/2024. 07-06 · Vehicle Requirements
7-06During the inspection, Licensing Specialist observed that the program's vehicle was monitored at time of inspection. The installed vehicle alarm was not in working condition in that it could not be heard from the distance of 500 feet from the vehicle. The director was given until 09/20/2004, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, regarding the maintaining of the vehicle alarm. CCF Handbook, Section 2.5.4, F - F. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 2. List of approved alarm manufacturers may be found on the Departments website at www.myflfamilies.com/childcare. 3. Alarms must be installed and maintained according to the manufacturers recommendations. 4. Alarm systems installed according to the manufacturers instructions prior to October 1, 2021, will be deemed to satisfy this requirement if they meet the criteria set forth in 2.5.4.F.1. above. Under this allowance, the existing alarm system must always be properly maintained in working order. Should at any time the alarm system require replacement, the new system must be selected from the Departments approved list referenced in 2.5.4 F.2. above (p. 17)

During the inspection, CCR observed on the school age playground that the fence in two different area at the base of the fence had a gap more than 3.5 inches off the ground. The first gap was 7 inches and the second gap was 4.5 inches. The director was given until October 20, 2023, to come into compliance with this standard. During the inspection, CCR provided technical assistance, CCF Handbook Section 3.5 F- Outdoor Play Area. F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3.5 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard.
