31-2: During the inspection, Licensing Specialist observed 1 childs file did not have a current immunization (exp. 7/1/2024). During the inspection, Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 8.1 A and B: The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C-22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department. 10-5: During the inspection, Child Care Regulation observed alcohol accessible to the children in care, in that the alcohol (Moscato, Moscato dasti, American Concord Grape, 3 Stella Rosa Loriginale, Stratusphere Gin, Crown Royal Regal Apple, and 3 unknown alcohol bottles) was being stored on the closet floor in the bedroom that the children sleep in. During the inspection, the provider locked the closet making the alcohol inaccessible to the children in care. This is being marked corrected at the time of the inspection.
