Observed the restroom that is shared between the toddler/ 2yrold class there is toilet stall that was missing the lid to the tank of the toilet. According to the Director the lid was broken so an universal sheet covering was added in place to cover the toilet lid. The Director made corrections by adding lid to the toilet during the time of inspection

Observed on video footage on 9/1/2023 during play time in the morning at 9:30am. The Child (DH) was playing with toy lawn equipment and fell Trip over the white PBC tubing protruding from the ground. This PBC Tubing is connected to a Toy fire hydrant water sprinkler. The two Teachers are on the play area but did not notice that child had fell. Also in the video footage when returning back into classroom the staff did not notice that child (DH) grabbing and rubbing her leg , limping and crawling while in classroom. The staff notice the limp in (DH) walk when the mother notice that the child did not walk towards her for pick up. According to the two teachers located on the play area: Yisell Rendon: stated that on 9/1/2023 she did not notice anything wrong with the child (DH) . Per Ms Rendon she saw the child playing in the center engaging with the other children and playing with toys . Ms rendon stated play time is from 9:30 - 10:00 am She saw the child (DH) playing and running around and her legs appeared fine. The child (DH) did not show any signs of hurt or made complaint about her legs. Aura Ortiz: stated that on 9/1/2023 the child (DH) appeared to be functioning normal. (DH) played with toys located on the outside and inside of the classroom. Per Ms Ortiz she saw (DH) walking / running and there was no complaints. According to Teacher that was inside classroom : Martha Marquez: Ms Marquez stated that (DH) appeared stable .The child (DH) played in the centers and grabbing toys. The child did not complaint or cry . Futhermore, Ms Marquez stated that all the chidlren were on the carpet playing. (DH) mom arrive to the classroom and the child begin to walk to her mother. Per Ms Marquez , when she saw the child walking to her mother she notice the child limping the same exact time the mother notice

Observed a teacher in the 1 year old classroom ( Freshman A) not washing her hands after diaperng a child. After diapering the child, the staff changed her gloves, sanitized the table and threw her gloves in the garbage. She then walked over to the children and held one child's hand. The person in charge K.P instructed the teacher to wash her hands
