05-04 · Technical assistance was given to the provider as a reminder that: Prior to transporting children by vehicle, the transportation log must be recorded with each childs name, the date and time of departure and initialed by child care personnel verifying that all children were accounted for and that the log is complete. Upon arrival at the destination by vehicle, the driver of the vehicle must: a. Mark each child off the log as the child departs the vehicle; b. Conduct a physical inspection and visual sweep of the vehicle interior to ensure that no child is left in the vehicle; and c. Record, sign, and date the transportation log immediately, verifying that all children were accounted for, and that the visual sweep was conducted. d. Ensure that a second child care personnel conducts a second visual sweep, signs and dates the transportation log verifying that all children were accounted for, and that the log is complete

07-06 · Technical assistance was given to the provider to have the alarm repaired/replaced prior to the due date. The provider had her mechanic come to the facility while the counselor was still present and repaired the system. That places the facility back into compliance for this standard
At the time of the inspection, the CCR counselor observed all of the facility's transportation vehicles. The vehicle with tag #NTZ-X45 alarm system activates, however, no audible siren. horn or other audio alarm activates that can be heard from 500 feet

12-19 · Facility Environment
At the time of the inspection, the facility did not have a daily indoor checklist. A sample daily indoor checklist was reviewed and emailed to the provider on today's date for use going forward. The provider was reminded that indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log and any problems must be corrected before the play area is used by children

23-04 · Technical assistance was given to the provider to obtain a tagged and inspected fire extinguisher rated at a minimum of 2A10BC prior to the due date. The provider was also reminded that the fire extinguisher needs to be mounted permanently in the event of a collision. The last fire drill was conducted on 2/29/24 and emergency preparedness drills were conducted on 2/28/24 (weather) and 12/26/23 (lockdown) The fire extinguisher(s) were last tagged in December 2023. A fire drill was conducted on today's date with the CCR counselor present. Note: the provider contacted the fire extinguisher company while the counselor was present and scheduled 2 new extinguishers to be delivered later today

32-07 · Technical assistance was given to the provider to add more mulch prior to the due date. A suggestion was given to the provider to add a border around the equipment to assist with containing the mulch and was reminded that if a border is installed, it must extend 6 feet away from the equipment in all directions. Note: the provider stated a delivery of mulch was canceled due to the city blocking the driveway on the day it was to be delivered. The provider has some mulch onsite, but was unable to be spread prior to the inspection
