During the time of inspection, Licensing Specialist observed holes with sharp edges in the white fence on the older children's playground. 32-07 · Outdoor Equipment
During the time of inspection, Licensing Specialist observed that the mulch on the toddler playground measured less than an inch. The mulch on the older children's playground measured 3 inches or less in areas around the slides and swings. CCF Handbook, section 3.12 Outdoor Equipment D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. 04-02 · Supervision
During the time of the inspection, the Director admitted that a child was left on the playground unattended between 10 to 15 minutes. The Director advised that another teacher went on break and found the child outside by themselves and brought the child back in. This standard was brought back into compliance. CCF Handbook, section 2.4.1 General Supervision Requirements B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group

39-03 · Accident/ Incident Notification and Documentation
During the time of the inspection, the Director advised the Licensing Specialist that she did not contact child's parents or document incident on the day that it occurred. The Director submitted the incident report via email to the Licensing Specialist on 7/31/24. This standard was brought back into compliance. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. 46-07 · Daily Attendance
During the time of inspection, the Director advised that the teacher did not check attendance. The Teacher advised the Licensing Specialist that she was filling in for another teacher who went on break, and she was not given an attendance binder. CCF Handbook, section 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. 17-06 · Outdoor Play Area/Fencing
During the time of the inspection, the Licensing Specialist observed a large gap in the fence located on the large playground. This violation was resolved and brought back into compliance during the time of inspection. CCF Handbook, Section 3.5 Outdoor Play Area F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided
