07-06 · Vehicle Requirements
During the time of inspection, the Licensing Specialist observed the 1 van used to transport children in care did not have working alarm which could be heard from 500ft. Per CCF Handbook Section 2.5.4, F, 1, d Vehicle Requirements F. By January 1, 2022, all vehicles used by childcare facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable

08-04 · Seat Belts/Child Restraints
During the time of inspection, the Licensing Specialist observed that the van did not have operable seat belts/restraints for the children in care. The Licensing Specialist advised the Provider that until standard brought back into compliance, the van was unable to be used for transporting children in care. Per CCF Handbook Section 2.5.5 Seat Belt/Child Restraints Each child, when transported, must be seated in a back seat in an individual factory installed seat belt or federally approved child safety restraint. The child safety restraint must be installed, secured and used in accordance with the manufacturers instructions and a copy of such instructions must be maintained (in the vehicle and/or on file). Child safety restraint must be replaced if they have been recalled, are past the manufacturers date of use expiration date, or have Child Care Facility Handbook Page | 18 been involved in a crash that meets the U.S. Department of Transportation crash severity criteria or the manufacturers criteria for replacement of restraints after a crash, as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by in 65C- 22.001(7)(v), F.A.C

During the time of inspection, the Licensing Specialist observed 4 personnel files did not have current record of Child Abuse & Neglect form. Licensing Specialist advised the Provider that form must be signed annually thereafter. Per CCF Handbook Section 7.4, C Personnel Records C. 30-06 · Diapering
During the time of the inspection, the Licensing Specialist observed the teacher that was temporarily in the infant room change a child's diaper in the crib on a non-impermeable surface. The Licensing Specialist addressed the concern regarding the diaper changing procedures with the Director and the infant room teacher. They informed the Licensing Specialist that typically the diaper changing takes place outside the classroom at a diaper changing area with an impermeable surface, but the teacher that had conducted the improper diaper change isn't typically in the infant room. The infant room teacher removed the items from the crib to clean and sanitize them and changed the crib sheets. According to the DCF Handbook 3.10.2 Diapering Requirements E. When children in diapers are in care, there must be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use

32-03 · Outdoor Equipment
During the time of the inspection, the Licensing Specialist observed the multicolored seat with springs located near the fence on the school agers playground to have 2 chairs that were cracked with sharp edges causing a concern for a child being cut while playing on the playground equipment. The Director has agreed to keep the children off the playground equipment until they can have it removed. According to the DCF Handbook 3.12 Outdoor Equipment B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months
