During time of inspection, Child Care Regulation observed transportation log for the school, year did not include a second sweep signature. Per staff transportation van is not in use during summer vacation. TA-A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable)

During time of inspection, Child Care Regulation observed missing outlet covers in the afterschool room. TA- All accessible electrical outlets must be tamper-resistant electrical outlets that contain internal shutter mechanisms to prevent children from sticking objects into receptacles. In settings that do not have tamper-resistant electrical outlets, outlets shall have safety covers. The standard was brought back into compliance when childcare personnel replaced all missing covers

17-02 · Outdoor Play Area/Fencing
During time of inspection, Child Care Regulation observed 2 wasp nests on the afterschool playground area under the wooden shading on the right-hand side. Also, the toddler playground area has a detached tarp low hanging from the shading area and a hole in the ground by the white fence over 3in in width. TA-The outdoor play area must be clean and free from litter, nails, glass and other hazards. Afterschool playground closed until wasp nest is removed

27-06 · Catered Food and Food Provided by Outside Sources
During time of inspection, Child Care Regulation observed the food Acceptance Log was not at facility for review. TA-If a facility chooses to provide or make available food to children in care from an outside source, such as a caterer, or a licensed child care facility under the same ownership that includes a food preparation area that meets licensing Child Care Facility Handbook Page | 33 standards, or as the result of a learning activity provided by a child care program, such as a garden, it is the responsibility of the provider to ensure all food intended for consumption by a child in care is free from spoilage and contamination and safe for human consumption. 1. A log must be maintained for all prepared meals being transported into the facility. The log must be retained for a minimum of 12 months. The log must include the delivery date, time of arrival, quantity and types of food, verification by the recipient of adequate temperatures of food, and the name and signature of the recipient. The facility shall not accept food that is not at the appropriate holding temperature. 2. If food delivered by an outside source does not meet licensing standards, the facility must have an alternate plan for meals. 3. Parents and legal guardians must be advised in advance of each food related activity, such as special occasions and learning activities that include food consumption. Written parental permission may be obtained in the form of a general or specific permission slip. Documentation of parent permission for food activities must be maintained for a minimum of 12 months from the date of each activity
