33-01 · Training Requirements
During the time of the inspection, the Licensing Specialist monitored the childcare personnel trainings and observed that one of the employees did not complete the 40hrs Introductory Training. According to the CCF Handbook, Section 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004, are not required to fulfill the competency examination requirement

CCRC observed live roaches, one was crawling on the counter top in the kitchen, and the other was on the fridge door magnet. TA was provided: Director informed CCRC that facility had a pest control service three weeks ago. Pest control was called. Pest control technician came during this inspection. Treatment was scheduled during the time of inspection. CCRC will return for reinspection on or after given due date. 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

24-18 · Food Preparation Area
CCRC observed a an expired milk stored in the fridge. The container was showing expiration date of 08/22/2023. TA was provided: Director discarded the milk. 3.9.2 Food Storage Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food-borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. Facilities choosing to prepare food must have a designated space for food storage within the designated food preparation area or in a room not calculated as part of indoor floor space, and in an area not used for diapering. Off-site food storage is permissible only if the site of storage is a licensed child care facility under the same ownership that includes a food preparation area that meets licensing standards. B. Stored food must be consumed or discarded on or before the expiration dates listed by the manufacturer

45-07 · Background Screening Documents
CCRC observed and an employee file, employee was added to the facility roster/clearing house on 2/03/2023, employee left the previous job/ childcare center on 7/6/2022. Center used the employee background screening that was done in 2/25/2021 and did not initial re- screening for this employee. CCRC observed the employee working with Two-year-old class, TA was provided: director submitted re-screening for the employee during the time of this inspection, Director merged the Two-year-old and the Three year-old classes for the the employee to be under direct supervision. Teacher must be always under direct supervision, and shall not have unsupervised contact the with children in care. until the re-screening result is clear. Director will send a copy of background screening result by given due date. 5.2 Re-Screening C. Child care personnel must be re-screened following a break in employment in the child care industry that exceeds 90 days. Child care personnel/individual with a break in service that exceeds 90 days are considered unscreened child care personnel/individuals until completion of re-screening. These child care personnel/individuals shall not have unsupervised contact with children in care
