12-01 · Facility Environment
During the time of the inspection, the Licensing Specialist observed the floors in the 2-year old room to be in need of cleaning, the walls and floors to be in need of cleaning in the 1-year old room. During the time of the inspection, the Licensing Specialist observed the transportation vehicles to be in need of cleaning. (trash and dirt build up on floor) During the time of the inspection, the Licensing Specialist provided technical assistance: per the CCF Handbook "A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months."

During the time of the inspection, the Licensing Specialist observed peeling paint in several areas of in the lunch/afterschool room 1, 1- year old room, and the afterschool room 3. During the time of the inspection, the Licensing Specialist observed covers to be missing from the overhead light fixtures in the lunch/afterschool room, 1-year old room, 3-year old room, and afterschool room 2. During the time of the inspection, the Licensing Specialist observed there to be water leaking from the ceiling into a large tin food can in the hallway leading to the kitchen area. There was water damage on the wall adjacent to the leak. During the time of the inspection, the Licensing Specialist observed rotting wood on the blue USA door and surrounding exterior wall and in various areas around the white awning over the 3 and 4-year old play area. During the time of the inspection, the Licensing Specialist provided technical assistance: per the CCF Handbook "A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. 32-03 · Outdoor Equipment
During the time of the inspection, the Licensing Specialist observed peeling paint throughout the entire outdoor area. (the outdoor floorspace, rocket ship, metal slide, around the movie building, swing set, and the large tube) During the time of the inspection, the Licensing Specialist provided technical assistance: per the CCF Handbook "B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. F. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. Any open containers with water must be emptied immediately after use, i.e. pots, toys, or other equipment that collects water."

33-09 · Training Requirements
During the time of the inspection, the Licensing Specialist observed 8 personnel records to be missing documented proof that they were trained on the use of the fire extinguishers. During the time of the inspection, the Licensing Specialist provided technical assistance: per the CCF Handbook "All child care personnel shall be trained in the use and operation of a fire extinguisher, at each facility they are employed, within 30 days of date of hire. 37-06 · First Aid Requirements
During the time of the inspection, the Licensing Specialist observed both of the vans to be missing the following items from the First Aid kits: soap, hand sanitizer, tweezers, scissors, bottled water. During the time of the inspection, this was resolved by the Operator adding the missing items to the First Aid kits. During the time of the inspection, the Licensing Specialist provided technical assistance: per the CCF Handbook "C. Each kit must be kept in a closed container and labeled First Aid and must, at a minimum, include: 1. Soap and hand sanitizer (to be used with supervision if hands are not visibly soiled and if no water is present), 2. Adhesive bandages, 3. Disposable, non-porous gloves, 4. Cotton balls or applicators, 5. Sterile gauze pads or rolls, 6. Adhesive tape, 7. Digital thermometer, 8. Tweezers, 9. Pre-moistened wipes, 10. Scissors, 11. Bottled water (for cleaning wounds or eyes), and 12. A current resource guide on first aid and CPR procedures."
