During the time of the inspection, the facility has 3 total toilets with 2 that are operational and 1 that is in need of repair. The toilet was located in the hallway for the three-four-year-old to use. The toilet had a broken flapper, and the Licensing Specialist was not able to flush the toilet. During the time of the inspection, the Licensing Specialist provided technical assistance: 3.7 Restrooms and Bath Facilities I. Each toilet and sink must be maintained in good operating condition, cleaned and sanitized or disinfected as needed, but at least once per day. Sinks utilized for teeth brushing activities/procedures by the children must be sanitized prior to the activity occurring

During the time of the inspection, the Licensing Specialist observed that the merry-go-round on the playground has a section in the middle with worn sharp jagged edges and it is accessible to children. During the time of the inspection, the licensing Specialist provided technical assistance: 3.12 Outdoor Equipment B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. During time of inspection, Child Care Regulation observed an unscreened volunteer was left alone with 8 (4 Four year olds, 2 Eight year olds and a Ten year old) afterschool children. TA- Child care personnel must be re-screened following a break in employment in the child care industry that exceeds 90 days. Child care personnel/individual with a break in service that exceeds 90 days are considered unscreened child care personnel/individuals until completion of re-screening. These child care personnel/individuals shall not have unsupervised contact with children in care. The standard was brought back into compliance when the volunteer was replaced with screened personnel

During time of inspection, Child Care Regulation observed electrical outlet covers were not in place in all rooms of the facility. TA-All accessible electrical outlets must be tamper-resistant electrical outlets that contain internal shutter mechanisms to prevent children from sticking objects into receptacles. In settings that do not have tamper-resistant electrical outlets, outlets shall have safety covers. 13-02 · Toxic Substances, Hazardous Materials and Weapons
During time of inspection, Child Care Regulation observed that refresher plug ins, Lysol cleaning gel, Clorox cleaning gel, plastic bags, Lysol spray, stain & sealer and other hazardous materials was not locked and was accessible and in reach of children by being stored in the bottom cabinet in the children's bathroom, outdoor play area and in the back classroom on the counter of the handwashing sink. TA-All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times. During time of inspection, Child Care Regulation observed less than 6 inches of mulch in the 6ft. fall/use zone under the stationary playground equipment. TA-Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. 35-11 · Communicable Disease Control
During time of inspection, Child Care Regulation observed the facility did not have a written exposure plan regarding safety precautions for exposure to blood and potentially infectious fluids. TA- There are three common modes of transmission for the spread of microorganisms in child care settings: contact, droplet, and airborne. Many common infections encountered in the child care setting are transmitted by direct or indirect contact. Child care facilities shall develop a written exposure plan regarding universal safety precautions, recommended by the Centers for Disease Control and Prevention (CDC), to follow in the event there is exposure to blood and potentially infectious fluids. Personnel are required to be educated on the facilitys exposure plan regarding standard precautions before beginning to work and annually thereafter. Written documentation that child care personnel have participated in the facilitys annual refresher education for the exposure plan and understand the proper procedures in the event there is exposure to blood and potentially infectious fluids must be retained for 12 months and be available for licensing to review
