During the time of the reinspection the Licensing Specialist observed the diaper changing table to have a pink sippy cup and a slack of lunch bibs placed on it. This was corrected at the time of the inspection as the teacher removed all items off the diaper changing table. During the time of the inspection the Licensing Specialist observed that the 2 white A/C Vents located on the back wall in the infant room are covered in dust. Per the DCF Handbook All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. During the time of the inspection the Licensing Specialist observed that 1 staff did not complete the 40 hour Introductory Training to meet the Child Care Training Requirement. Per the DCF Handbook Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. During the time of the inspection, the Licensing Specialist observed that the lattice around the bottom of the fence area is cracked with jagged edges. The metal fencing needs to be secured with supports to prevent the gate from leaning, also the fence area in the playground area is missing the top support leaving the metal area exposed with a sharp edge. During the time of the inspection, the Licensing Specialist observed the outdoor playground area toys to not be clean or in good repair. Licensing Specialist observed some toys to have cracks, dirty and with leaves and spiderwebs inside and around the toys. No documentation of the indoor or outdoor play area inspection. 3.1 General Health and Safety Requirements A. All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. During the time of the inspection, the Licensing Specialist observed that there was two bottles of aspirin and one bottle of Advil in the draw of a clear plastic stacked storage container accessible to the children without a child safety lock. The Director moved the bottles of medication to the office area that is not accessible to the children. The facility is back in compliance for unsafe storage of dangerous materials. 23-01 · Fire Drills
During the time of the inspection, the Licensing Specialist observed that the annual fire inspection expired on 1/18/24. The licensing Specialist was advised by the Director and Owner that the annual fire inspection has not be conducted. According to the childcare facility handbook 3.8.2 Fire Safety A. Unless statutorily exempted, all childcare facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Childcare Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. 24-09 · Food Preparation Area
During the time of the inspection, the Licensing Specialist observed that the hot running water was not a minimum of 100 degrees Fahrenheit. The Director called the maintenance man during the time of the inspection. The maintenance man arrived on site and showed the Director that the valve on the hot water heater that allows for the hot water at this sink had somehow accidently been turned off. The maintenance man turned the valve back on and the facility is now in compliance for hot running water at the handwashing station. According to the childcare facility handbook 3.9 Food Preparation/Food Service 7. A separate handwashing station with hot running water, a minimum of 100 degrees Fahrenheit. The handwashing station must include a sink with running water and drainage, soap, trash can, and disposable towels or hand-drying machines that are properly installed and maintained. A handwashing sink shall not be used for any purpose other than handwashing. Handwashing stations must include posted signs visible to child care personnel and children demonstrating proper handwashing technique. Portable sinks may not be used for dishwashing or food preparation. If a portable sink is used for handwashing in the food preparation area, hot water must be provided

24-18 · Food Preparation Area
During the time of the inspection, the Licensing Specialist observed that there was several packages of expired food in the kitchen/food preparation area. The Director disposed of the expired foods into the trash receptacle. According to the childcare facility handbook 3.9.2 Food Storage Proper storage of food is essential to prevent food contamination, as well as insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food-borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. Facilities choosing to prepare food must have a designated space for food storage within the designated food preparation area or in a room not calculated as part of indoor floor space, and in an area not used for diapering. Off-site food storage is permissible only if the site of storage is a licensed childcare facility under the same ownership that includes a food preparation area that meets licensing standards. B. During the time of the inspection, the Licensing Specialist observed that three children had expired immunization records in their file. According to the childcare facility handbook 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. A. The parent/guardian of a child who has not received the age- appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations. C. School-aged children attending public or non-public schools are not required to have student health examination and immunization records on file at the childcare facility as such records are on file at the school where the child is enrolled. D. If the custodial parents or legal guardians need assistance concerning these requirements, the facility shall refer them to the Department of Health or to the childs physician. E. Medical records in this section are the property of the custodial parent or legal guardian and must be returned to them when the child withdraws from the facility. It is recommended that childcare personnel are current with all immunizations routinely recommended for adults by the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention (CDC), as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by in 65C-22.001(7)(v), F.A.C

During the time of the inspection, the Licensing Specialist observed that the health form in one child's file is expired. Accoing to the childcare facility handbook 7.2 Student Health Records The childcare facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. Any child who has or is at an increased risk for a chronic physical, developmental, behavioral or emotional condition and requires additional services must have a current Emergency Care Plan, prepared by the parent/guardian or physician, included in the childs file and readily accessible for those caring for the child.
