During the time of the inspection the Licensing Specialist observed that the changing tables in the Infant room and the Two's classroom had dirt under both diaper changing mats. This was corrected the time of the inspection as the teacher in each classroom cleaned and sanitized under each diaper changing mat. Per the DCF Handbook When children in diapers are in care, there must be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use

During the time of the inspection the Licensing Specialist observed that 1 staff record did not have a current Abuse and Neglect Reporting form. This was corrected during the time of the inspection as the staff signed a new form dated 4/8/25. During the time of the complaint inspection the Licensing Specialist observed 2 bottles that were not labeled with a child's first and last name. This was corrected at the time of the inspection as the teacher in the classroom labeled both the bottles with the child's first and last name. Per the DCF Handbook Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily

04-01 · Supervision
During the time of the inspection, Licensing observed the school aged and four year old teacher standing outside of her classroom, standing in the hallway, leaving the children in the classroom without direct supervision. This was resolved during the time of the inspection. The teacher went back into the classroom to properly supervise the children. During the time of the inspection, Licensing provided technical assistance: "Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group. D. At all times, lighting must allow child care personnel to see and supervise children while in care. E. A program is responsible for the supervision of a child until an authorized individual retrieves the child from the program. A child shall not be released to any person other than the person (s) authorized or in the manner authorized in writing by the custodial parent or legal guardians. All individuals authorized to pick up a child must be identified in writing prior to release by the custodial parent or legal guardian to the program, and the program must verify the individual picking up the child is authorized by using a picture form of identification. Each child transported must be dropped at the designated location as agreed upon by the provider and the custodial parent/legal guardian and released to an authorized individual."

17-06 · Outdoor Play Area/Fencing
During the time of the inspection licensing specialist observed that the fencing along the backside of the playground had two spots where the screws were missing that attach the fencing to the post. According to the DCF handbook 3.5 Outdoor play area. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals

19-02 · Nap/Sleep Space Requirements
During the time of the inspection licensing monitored that a distance of 18 inches was not maintained between each napping/sleeping space. This was corrected at the time of the inspection. According the the DCF handbook 3.6.2 Napping/sleeping space A minimum of 18 inches must be maintained around individual napping and sleeping spaces. A maximum of two sides of a napping or sleeping space maybe against a solid barrier, such as the wall. The solid side of a crib does not meet the requirements for a solid barrier. Napping and sleeping spaces must not be under furniture or against furniture that creates a hazard. Napping and sleeping areas must not be in exit areas. All exits must remain clear in accordance with fire safety requirements

24-23 · Food Preparation Area
During the time of the inspection the licensing specialist observed that the refrigerator did not have a thermometer. According to the DCF handbook Refrigerators/freezers: An accurate thermometer designed to measure cold storage temperature must be placed inside each refrigeration and freezer unit. Thermometers in refrigerators must show a reading of 41 degrees Fahrenheit or below, and thermometers in freezers must show a reading of 0 degrees Fahrenheit or below. The thermometer must be located in the center of the unit and be readily accessible. Thermometer temperature readings higher than specified above require further temperature testing of food samples stored in the unit using a probe type thermometer; and adjustments to the unit setting to reach and maintain the required readings must be made

32-03 · Outdoor Equipment
During the time of the inspection licensing specialist observed the jungle gym has rotten and loose wood planks with exposed screws. The railings were also loose. The equipment was closed and taped off from the children in care. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children.
