On 2/6/2025 a child was left in a classroom inside the facility without childcare personnel supervision. The provider self-reported and sent video footage of this incident. 2.4.1 General Supervision Requirements B. Licensing Specialist observed an attendance sheet check list for the day of the incident. Licensing Specialist observed that the child had a check mark next to his name under the playground column stating that the child had entered the playground but the child was left in the classroom alone instead. TA: Licensing Specialist discussed requirement with the provider. 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. B. The custodial parent or guardian may document the time when his/her child enters and departs the child care facility or program. However, child care facility personnel are responsible for ensuring that attendance records are complete and accurate. C. REMOVED. D. Attendance records must be maintained for a minimum of 12 months. During time of inspection, Child Care Regulation observed electrical outlet covers were not in place in the summer camp room. During time of inspection, Child Care Regulation observed facility with expired fire inspection. Fire inspection expired on 5/17/23. TA- Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A- 36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. During time of inspection, Child Care Regulation observed fire drill log was missing May 2023. The last fire drill was completed on 6/14/23 at 3pm, utilizing the primary route. There were 4 personnel, and 17 children present during the drill. It took 1 minutes to complete the drill. The fire extinguishers were serviced on 7/23. During time of inspection, Child Care Regulation observed less than 6 inches of mulch in the 6ft. fall/use zone under rope climb. TA-Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment.
